To prepare for your first appointment you and we will need to complete a few tasks. On this page you will learn what these tasks are and what steps are needed to get them done together!
We will always talk to you on the phone to coordinate your first appointment. John, our office manager, will likely be who you talke you. During this phone call we will cover the following:
You will be required to complete and submit several documents, some will be familiar like a privacy form, and others will be specific to the reasons you're coming in.
Please complete as soon as you can, Dr. Padilla needs to review your symptom information before your appointment so that she can be well-prepared for your appointment. She asks that you complete these forms no later then two days before your appointment, though sooner is very helpful.
In the forms you will also find a map with instructions on how to find our office.
Please review the map and directions included in your new client forms packet. Our location is in the back of an office park and can be a bit tricky to find.
Please give yourself plenty of time to find us. If someone is 10- or more minutes late then the appointment will be rescheduled in order to allow for the necessary time to complete the appointment. So please do call us if you need help getting here :)
Once here you will likely be welcomed by John and he will walk you thorugh how to ring Dr. Padilla's office doorbell when you arrive to your future appointments, as well as show you where you can find the restroom and offer you a bottled water.
